Fatal BP blast leads to safety documentary

As the third anniversary of one of the worst industrial accidents in U.S. history approaches, the series of blasts that rocked Texas City are still being felt in ways few could have imagined. The explosion killed 15 workers and injured 180.
The lead federal agency to investigate the fatal blasts has taken the extraordinary step to create a documentary-like video of the lessons learned from the disaster.
Called “Anatomy of a Disaster,” it is a moment-by-moment account of the mistakes leading up to the 2005 explosion.
It also includes much of the video from 11 News’ coverage of the explosions.
“This project is unique in the history of the Chemical Safety Board, because this is our most comprehensive and detailed investigation we've ever done,” said Daniel Horowitz of the Chemical Safety Board.
Using interviews from its own investigators and independent experts, the CSB concludes the explosion was the result of cost cutting, production pressures and the failure of BP to invest in the refinery's infrastructure.
“There's an old saying, if you think safety is expensive, try an accident. Accidents cost a lot of money,” safety expert Trevor Kletz of Texas A&M says in the video.
For BP, the cost has been billions of dollars and the CSB says all industries could learn from the company's mistakes.
Lessons not just for those in the petrochemical field, which is why the CSB is making the video available to anyone who wants it.
“We want these lessons to be studied and understood by business all around the world,” said Horowitz. “And if any good can come out of the terrible loss of life three years ago, it is that we all will take steps to prevent similar accidents from happening again and again.”
