HOUSTON -- Many mistakes were made during last year's massive egg farm fire in Wharton County, according to a new report by the state fire marshal's office.

Firefighter Cpt. Thomas Araguz died in the July 3 blaze. The report says firefighters did not fully size up the building before going in, and they didn't check to see if backup crews were outside to rescue them if needed.

Investigators say once inside Araguz and another captain walked away from the hoseline, which was their only line to the outside.
