Police became involved in separating Brooke Mueller from her 7-year-old twins with Charlie Sheen on Wednesday.
Police released a statement through the Utah Department of Public Safety's website saying a woman claiming to be the children's grandmother called police and requested assistance.
Police tracked down Mueller's car on a highway in a "welfare check," and sent Mueller to a local hospital.
The statement reads: "On the morning of November 16, dispatchers with the Utah Department of Public Safety received a welfare check call from a woman claiming to be the mother of Brooke Mueller. Troopers with the Utah Highway Patrol responded to the welfare check call and located Mueller, her two children and a nanny on Interstate 15 near Nephi, Utah."
Twins Max and Bob were then separated from Mueller, 39, according to the statement. "The children are safe and will be released to family. Mueller was taken to an area hospital for evaluation. The State Bureau of Investigation is currently investigating the incident."
There is no mention of Sheen in the police statement at this time. Mueller and Sheen were married from 2008 to 2011. The two have subsequently battled over custody of the twins, and both parents have struggled with addiction issues.
For a brief period in 2013, the twins were under the care of Sheen's second ex-wife, Denise Richards.
